You should be able to check the exact settings differences by going into sandbox and selecting your saved sandbox presets dropdown at the bottom. There are no NPC's anymore in vanilla, HOWEVER, there is a mod you can get that adds NPCs to the game. Multiplayer is available but it's outdated in an old build. THe latest build should recieve MP by the end of the year I've heard. Superb survivors adds npcs. Not perfect but it'll definitely scratch that itch.

And the best settings I found for me so far are, suvivors spawn rate at the lowest. 5% or 0% chance of spawning with guns (because there is always a guy with a shotgun making noise at 2 in the morning attracting zeds). And the real hard setting is 6 monthes later...not Apoclypse. For a first time player, Survivor (or perhaps even Builder) is probably better.
